Visit the Social Security Administration website (www.ssa.gov) and follow these 5 simple steps to submit a disability statement.

  1. Click on the Apply for Disability Benefits Link
  2. Review the Adult Disability Checklist for details on what you will need before beginning the online process.
  3. Complete the online Disability Benefits Application to provide the Social Security Administration with information about eligibility for payment
  4. Complete the Adult Disability Report online to provide the Social Security Administration with medical and employment history
  5. Complete, sign and mail the authorization to release information to the Social Security Administration or take it to the nearest Social Security Administration office.

The advantages of applying for online social security benefits are advantages, not only do you not have to wait for an appointment from the SSA, but you can also start your Social Security disability application immediately and find out if you meet the qualifications faster.

You can file a disability application online if you are 18 or older, have worked and paid Social Security taxes long enough to qualify for Social Security Disability Insurance, live in the United States or one of your territories and your Mental or physical health is expected to last for at least 12 continuous months or end in death.

  • Learn how to present a disability by phone or at the local office. Call the toll-free number of the Social Security Administration at 1-800-772-1213 (TTY 1-800-325-0778) between 7 a.m. and 7 p.m., Monday through Friday to schedule an appointment.
  • Presenting the disability in person by visiting your local Social Security office.
